Autism Spectrum Disorder (ASD) referral pathway

Information to support you in making referrals for Children and Young People (CYP) in educational settings to the Community Paediatric Teams in Hertfordshire (West Herts and East & North Herts):

  1. Guidance for making referrals
  2. ASD pre-school/school questionnaire (completed by schools)
  3. ASD parent/carer pre-assessment information (completed by parents with support from school)
  4. ASD referral forms (completed by GP/health professional once parent and school information received)

Prior to considering a referral to the Community Paediatric Team for ASD investigations, please ensure all parents/carers are offered support and advised to contact local support groups to access further information, parent training and an educational programme.

Referrals are accepted from GPs but rely on information from parents, professionals and school (when the child is in a school setting). When a child or young person is in a school setting, the referral should be initiated by school/educational setting with the ASD school questionnaire being completed and supporting family to complete the parent Information. The family will then go to the GP to complete the referral process.

Please note: From the 15th February 2021, referrals for ASD investigation will only be accepted on these updated referral forms. Where incomplete referrals are received, they will be returned to the referrer so as all information can be provided.
